If you take medications that require ongoing monitoring or regular prescriptions, please make arrangements with your new provider before the practice closes.
Prescription requests and other clinical needs will continue to be handled on a case by case basis during the transition period, but I strongly recommend that patients not wait until the final days of the practice to establish new care.
Closing my practice was not a decision I made lightly. It has been a privilege to care for my patients, and I sincerely appreciate the trust you have placed in me and The Sage Clinic.
